health anxiety: when every symptom is a verdict

Health anxiety is the one where the doubt is never absurd. People do get sick. The lump sometimes is something. That's what makes this theme so much harder to argue with than most, and it's why the usual reassurance, you're fine, you're young, stop googling, slides straight off.

what it looks like from inside

A sensation turns up. A twinge in your side, a headache that's lasted two days, a patch of skin that looks different than it did. Most people register it and move on. You can't, because a question has opened and it feels answerable, and answering it has become the most urgent thing in your day.

You check. And checking works, for about four minutes.

the two directions it goes

Some people check constantly. Searching symptoms, pressing the spot to see if it still hurts, taking their pulse, photographing a mole weekly to compare, booking the appointment, then booking a second opinion on the appointment.

Others go the opposite way and avoid entirely. No physicals, no screenings, no doctors, because as long as nobody looks, nobody can tell you. That version gets read as carelessness by everyone around them. It's usually terror.

Plenty of people do both, in the same month, about different body parts.

why your doctor can't fix this

A clear scan brings enormous relief. For a day, sometimes a week. Then the mind produces the obvious follow-up, which is that scans miss things, or that it was taken before whatever it is started, or that you described the symptom badly and got tested for the wrong thing.

This isn't a failure of the doctor or of your intelligence. Certainty about your own body simply isn't purchasable. Every test is a snapshot, every result is a probability, and a mind looking for a guarantee will find the gap in any of them. The relief from a normal result is real. It's just rented.

an important thing about treatment

Treatment for health anxiety is not about ignoring your body, and I want to be clear about that because it's the reasonable objection.

You keep your annual physical. You get the screenings appropriate for your age and history. If something really changes, you get it looked at, once, the way any sensible person would. We aren't building a person who ignores a symptom out of principle. That would be its own disorder, and a more dangerous one.

What changes is the compulsive layer on top. The eleventh search. The fourth doctor. The daily palpation of the same spot, which, as it happens, is why the spot still hurts.

what the work actually targets

ERP for health anxiety usually starts with an honest inventory of the checking, because most people have lost track. Then we pick somewhere to start: leaving a sensation unexamined for an hour, then a day. Closing the browser with the question open. Going to the appointment you scheduled, and not the two you added afterward.

Some of it is uncomfortable in a specific way, because you're being asked to let a possibility stand unresolved, about your own body, which is the thing you have the least ability to walk away from. That's why we go in an order you choose and at a pace you set.

What we don't do is try to convince you that you're healthy. I don't know that, you don't know that, and pursuing that knowledge is the activity that's been running your life. The target is the pursuit.

who this is for

People who've been called a hypochondriac, which is a word that describes the behavior and explains nothing. People who've burned through three practices and can hear the tone shift when they call. People who stopped going entirely and know that's worse. And people whose health anxiety started after something real happened, to them or to someone they loved, which is extremely common and changes nothing about whether this is treatable.

common questions about health anxiety

how is health anxiety different from just being careful?

Care is proportionate and it ends. You notice something, you get it checked, you accept the answer, you go back to your life. Health anxiety doesn't end. The answer doesn't hold, the question reopens with a new angle, and the checking expands over time rather than resolving. The clearest tell is what happens after good news: relief that lasts days rather than settling the matter.

is health anxiety a form of OCD?

It runs on the same machinery, an intolerable doubt plus compulsions that bring brief relief, and it responds to the same treatment. Diagnostically it may be described as illness anxiety disorder, somatic symptom disorder, or OCD with health themes depending on the presentation. The label matters less than the mechanism, which is identical.

what if I actually am sick and therapy makes me ignore it?

Right question. Treatment keeps normal medical care in place. Annual physicals, age-appropriate screenings, and getting a real change looked at all continue. What gets targeted is the compulsive layer: repeat searching, repeat appointments for the same reassurance, and constant body checking, which frequently creates or worsens the very sensations being monitored.

my anxiety started after a real diagnosis. is this still the same thing?

Often yes, and it is very common. A real medical event teaches the nervous system that bodies can betray you without warning, and the vigilance that follows makes complete sense. It can still become a loop that outlives its usefulness, and it responds to the same work, sometimes alongside trauma treatment.
